Service Description:
|
Provides short term client centered mental health case management from an integrated model of care. This includes, information and education around mental health, substance use, concurrent disorders, advocacy, care coordination, referrals to appropriate programs/services and groups.

Application:

Referrals are accepted from anyone, including self-referrals.
To connect with this program, individuals are asked to come into the drop-in program during program hours. No phone calls.
