Service Description:
|
Provides a continuum of supportive housing to those who are impacted by mental health and are homeless or at risk of becoming homeless. It utilizes a peer support model to encourage natural support
networks and includes clients in the decision-making process. High support site provides 24/7 personal and residential support services to adults of all ages living with complex mental health and/or
problematic substance use, with a high number of hospital admissions and emergency department visits.

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
