Service Description: Single entry point for providing access to geriatric mental health beds at Baycrest and the Toronto Rehabilitation Institute. All referrals are triaged through the Toronto Region Behaviour Supports Coordinating Office housed at Baycrest. 

For consideration of admission to the Baycrest Behavioural Neurology Unit or Toronto Rehabilitation Institute's Specialized Dementia Unit, patients must first be seen and followed by the Virtual Behavioural Medicine (VBM) program. Eligibility / Target Population: Adults 65 years and older (exceptions to young individuals applied by specific sites) who are medically stable, have a long standing or newly diagnosed mental health and addiction disorder, concurrent disorder, Alzheimer's disease or related dementia and are referred by a health care professional * please see specific eligibility criteria for each program
